Welcome to Community Active & Wellbeing CIC.
If you’d like to join our sessions, you can self-refer quickly and easily using the steps below.
Our sessions are designed to help you improve your health, fitness, strength, mobility and confidence in a supportive and welcoming environment. We are inclusive of all abilities, including people who are new to exercise, returning after time away, or managing a long-term health condition.
Self-referral is the best option if you are signing up for yourself (or supporting a family member/friend to join) and you are ready to get started.
STEP 1
COMPLETE THE HEALTH DECLARATION FROM
Before attending your first session, we ask all new customers to complete a short Health Declaration Form.
This helps us understand your needs, identify any support or adaptations you may benefit from, and ensure we deliver sessions safely.
STEP 2
COMPLETE OUR MEMBER RECORD REQUEST
Please fill in the form below to register your interest in joining Community Active & Wellbeing CIC. Your member record will then be created on our system.
Community Active & Wellbeing CIC use Coacha to manage and administer their session's admin. All of your information is securely locked in the cloud. Please try to ensure all information is correct and accurately entered.
IMPORTANT SAFETY INFORMATION
For most people, physical activity is safe and beneficial.
However, if you answer YES to certain health screening questions, we may recommend that you speak to your GP/healthcare professional before participating, or we may adapt your exercise options for safety.
If you feel unwell or experience urgent symptoms, please contact your GP, NHS 111, or call 999 in an emergency.
WHAT HAPPENS NEXT
Once you’ve completed the steps above:
✅ Your information is received securely
✅ If anything needs clarification, we will contact you
✅ You’ll be ready to attend your first session
At your first session, your coach will welcome you, explain what to expect, and provide options or modifications if needed—so you can exercise at a level that feels right for you.
WHATSAPP GROUP UPDATES
We may offer WhatsApp groups for session reminders, updates and cancellations.
Please note: if you join a group, your mobile number will be visible to other members.
